Support Coordination

What is Support Coordination

Your personal NDIS navigator.

Support Coordination is a capacity-building support funded under the NDIS. Think of it as having an experienced guide who knows the system inside out - someone who helps you turn your NDIS plan into real-life support that actually works for you.

Getting Started

Step

We sit down with you to understand your goals, your current situation, and what you want to achieve. No jargon, no pressure - just a clear conversation about what matters to you.

Building Your Roadmap

Step

We create a personalised support plan that maps out which providers you need, how your funding will be used, and what milestones we are working toward together.

Ongoing Support

Step

We stay with you through the journey - checking progress, resolving issues, adjusting supports, and preparing you for plan reviews so you never feel lost in the system.

Three Support Levels

Support that grows with your needs.

The NDIS funds three levels of support coordination. We help you access the right level based on your situation.

Support Connection

Short-term guidance

Foundational support that helps you understand your plan, connect with community and funded supports, and build basic confidence navigating the NDIS.

  • Understand your plan and funding
  • Connect with community supports
  • Build early navigation skills

02

Provider Connections

We connect you with NDIS providers who match your needs, preferences, and location. You get to choose who works with you, and we ensure all providers are registered, reliable, and person-centred in their approach.

Plan Reviews & Advocacy

When your plan is up for review, we help you prepare, gather evidence, and advocate for the supports you need. You never have to face the NDIS alone - we are with you every step of the way.

04

Building Your Confidence

The ultimate goal of support coordination is to build your skills so you can manage your own plan with confidence over time. We work ourselves out of a job by empowering you to take control.

Who It Is For

Is Support Coordination right for you?

If any of these describe your situation, Support Coordination could make a real difference in how you experience the NDIS.

New to the NDIS

Your first plan feels overwhelming. You need someone to explain it in plain English and help you take the first steps.

Going through change

Your situation has shifted - moving homes, changing health needs, or aging out of school supports. You need someone to help you adapt.

community

Managing multiple providers

You have several support workers, therapists, and services. Coordinating them all takes time and energy you do not have.

Complex or high needs

Your situation involves multiple systems - health, housing, justice, child protection. You need a specialist coordinator who can navigate complexity.

Not sure if you have Support Coordination funding?

Check the Capacity Building section of your NDIS plan. If you see "Support Coordination" listed with an amount, you are funded and ready to get started.

Level 2 (Coordination of Supports) helps most participants implement their plan, connect with providers, and build capacity. Specialist coordinators hold allied health qualifications.

Support Coordination is typically funded for 12 months alongside your plan. At your plan review, you can request ongoing funding if you still need help coordinating supports. The goal is to build your skills so you eventually manage your own supports.

Yes. You have the right to choose your providers under the NDIS. If your current coordinator is not the right fit, you can switch to us at any time with no penalties. We make the transition smooth and seamless.
